The PROVINCIAL GOVERNMENT OF AGUSAN DEL SUR (“we, us”) is committed to protect and respect your personal data privacy.
In order to protect the health of our constituents in the threat of the COVID-19 pandemic, we are gathering information to process their entry.
We are at the forefront of not only implementing but also complying with RA No. 11332 or the Mandatory Reporting of Notifiable Diseases and Health Events of Public Health Concern Act, the provisions of the Data Privacy Act of 2012, and its Implementing Rules and Regulations (IRR) with respect to gathering information to help the Contact Tracing of Close Contacts of COVID-19 Cases.
We are committed to protecting the health and well-being of our constituents. In order to protect the health of local citizens of the province, we are collecting your information for contact tracing.
You will be asked to provide basic information including your full name, gender, birthdate, address, employment information, place of origin, and health/medical history, travel history, and other information. Upon entry to an establishment, your QR code is scanned and your data along with the name of the establishment and your temperature will be collected. Usage Data are also collected whenever your QR code is scanned. The Usage Data may include information such as the GPS location of the establishment you entered and the time and date of entry.
Your information will be used for the monitoring of entries in establishments within the province used for contact tracing, information dissemination, and analysis on valuable information used for our fight against COVID19.
Your data is encrypted and secured on computers within the provincial government data center.
Your information might be disclosed to DOH, other agencies, and authorized persons to provide an effective response during this COVID-19 pandemic.
The data collected will be retained for the duration of the COVID19 monitoring and will be archived and disposed after the duration of retention is over.
We do not authorize any entity to process your information. Any information provided by you to other entities in the creation of a QR code is not under our security and protection. We are not liable for any misuse of your information in the hands of outside entities.
By entering the information asked on the form, you understand and agree to the use of the Provincial Government of Agusan del Sur to process and disclose your data to other parties within the bounds of law mentioned above.
We recognize your rights in accordance to the Data Privacy Act of 2012. You may contact the Provincial Information Management Office for any inquiries regarding these rights.
